Aside from that visit from guys with guns, the other thing that made me want to leave was the fact that despite getting "excellent" performance evaluations, I was still doing the same things. There just wasn't any opportunity to move up the career ladder, get more responsibility, etc.
That's the curse of working in IT, the only way to get promotions is to change jobs.
