It looks as though the guy in the middle was doing some sort of stunt, seeing that the camera is centered on him.

I'll bet that they were about to take a picture of him, and then "Mr. Puke" just up and went, "BLEARRRRRGH!"

Yeah. Also notice the beer or whatever drooling out of the mouth of the guy on the right. I think you're right. They were probably doing some beer drinking stunt and it was the middle guy's turn when the one on the left let loose.
